Intermediate Care after hospital discharge

We provide post operative care following elective or major surgery, helping to ease the transition for patients who prefer to recover in their own homes.

Our aim is to help you achieve your recovery goal plans, promote your independence and reduce your dependency on care. We work with health and social care staff to ensure that you receive the care and support that is right for you and reduce the risk of re-admission.

Our fully trained and friendly staff will support you to follow recommendations for rehabilitation and help you to regain your confidence.

Enablement services are usually provided for limited periods, sometimes for no more than six weeks.
