NHS Continuing Care at Home
Our aim is to improve the quality of care at the end of life and to enable our service users to live and die in the place of their choice. We work in accordance with the NHS End of Life Programme.

We provide enhanced care for people with high individual support needs. This includes people who have Dementia or Alzheimer's, Stroke, Parkinson's, COPD and other long term conditions.
Our nurses and health care assistants can provide more intensive care and assistance with tasks such as Stoma Care, Specialised Feeding (PEG) and Medication Administration.
Our Nurse Manager will visit you for a free no obligation assessment that will ensure you receive the appropriate care that is designed for your own comfort in the surroundings of your own home.
Following clinical assessment, you may be eligible for continuing care funding by the NHS if you meet their criteria for funding.
We can offer live in care for short term or as long as determined by your assessment.
